Type your answers, sign with your finger or your mouse, attach your records, and the whole thing becomes one PDF you email to the office. Nothing is uploaded to us along the way.
Your answers, your signature and your files never touch our website. The PDF is built right here in your browser and saved to your device. You then attach it to your own email, so you can see exactly what you are sending before you send it.
For the school year beginning Monday, August 24, 2026.
Leave blank if your child has not been to school yet.
If applicable.
The school sends class reminders: event days, early dismissals, the monthly assembly, uniform days. Tell us where to send them and we will not send anything else.
Sign with your finger on a phone or tablet, or with your mouse. This signature is drawn into the PDF.

Optional, and you can always bring these in later. Photos and PDFs are added to the end of the same PDF, so the office gets one file.
Check your downloads for the file. Now email it to the office and attach that file.

Paying the fee now is optional, but it saves a step. You can also pay at the office.
The office will still need anything you did not attach: immunization records, a physician’s report, a recent report card, a photograph and a recommendation form. Questions, call (760) 345-8100.
Plus a student information sheet, emergency authorization, and a signed handbook acknowledgment. We hand you those at the office.
Immunization records and a physician’s report of a recent physical examination.
A recent report card, a recommendation form, and a photograph of your child.
Bring what you have and we will help you finish the file. Questions? Call (760) 345-8100 or come by, 42-675 Washington Street.
